Inspector Notes

A renewal inspection was initiated and concluded on 2/21/2024.There were 72 children present with 15 staff supervising and 2 administrators. The children were observed during outside time, teacher led activities, and engaged in various centers in the classrooms. A total of 7 child records and 5 staff records were reviewed. The inspection started at 8:35 am and concluded at 10:35 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

(Repeat) Based on record review and staff interview the center did not ensure they had a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The director confirmed that Child #5 (start date 9/6/2022) did not have documentation of a written care plan that included the food to which the child has a confirmed food allergy.
Plan of Correction: Parent filled out the written care plan from the doctor. Going forward, we will use licensing forms instead of the doctor's action plan to ensure we have all required info.
