Inspector Notes

An unannounced monitoring inspection was conducted March 21, 2024 between the hours of 11:39am to 1:50pm. There were 18 children directly supervised by 4 staff. The physical plant, 3 staff records, 5 children?s records,evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outdoors, transitioning inside from outside and eating lunch. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on observation, hazardous substances such as cleaning materials were not kept in a locked place.
Evidence:
One bottle of cleaning spray was observed in a lower unlocked drawer in the 1st Pre-Kindergarten classroom.
Plan of Correction: The cleaning spray was placed in a locked cabinet at the time of inspection.

Based on record review, injury reports did not contain all required information.
Evidence:
1) The incident report forms used by the center did not contain an area to document two signatures. Only one signature was observed on each form reviewed.
2) The incident report forms did not contain an area to document the staff present when an injury occurs.
3) The incident report forms did not contain an area to document any future action to prevent recurrence of the injury.
Plan of Correction: We will modify the VDOE injury report template with our letterhead.
