Inspector Notes

An unannounced monitoring inspection was conducted on April 16, 2024 from approximately 2:45 pm to 3:40 pm. Two staff were present with nine preschool age children. Children were observed finishing snack, hand washing and returning to classroom activities. Two staff and five child records were reviewed. Inspector reviewed standards in the areas of administration, physical plant, programming, medication, emergency supplies, emergency response drills and medication. Violations found are noted on the violation notice.

Evidence: The record for child #5 (start date 3/18/2024) did not have an address for an emergency contact.
Plan of Correction: Staff will get the information from parent today.

Based on interview, facility did not ensure 1 of 2 staff records are maintained and kept at the center.

Evidence: The record for staff #2, present during inspection, was not available for review. Owner stated she had taken the record home.
Plan of Correction: Facility will ensure staff records will be kept at the center.
