Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on August 28, 2024. The on-site inspection began at 10:05 am and ended at 11:35 am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on review and interview, facility did not ensure out of state background checks are completed for a staff member that has lived outside the state of Virginia within the past 5 years.

Evidences: The record for staff #1 (start date 5/1/2024) had documentation of living in another state within the past 5 years and there was not documentation of completed sex offender check and child abuse/neglect registry check for those states. Staff confirmed they were not completed.
Plan of Correction: All background checks were completed and submitted on day of inspection.
