Inspector Notes

An unannounced monitoring inspection was conducted today from 9:40am-10:30am with the provider on site. There were no children present. A sample of 1 children's files and 11 caregiver/household files were reviewed. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, drill records and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Based on review of 11 caregiver/household members' records, the provider did not obtain an updated central registry report every five years for (4).caregivers/household members.
Evidence:
1. Household Members #1, #2, #3, #4 all had Central Registry reports dated 8/17/2018 and needed to rechecked.
