Inspector Notes

An unannounced monitoring inspection was conducted on 06/17/2024 from 8:30am-9:30am. There were 6 children in care (14 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely. Areas of non-compliance are identified in the Violation Notice.

Standard 22.1-289.035-A
Based on record review, the provider did not obtain a fingerprint-based criminal history record check every 5 years for 1 caregiver.

Evidence:
1. The most current fingerprint eligibility letter in the record for Caregiver #1 was dated 05/09/2019.
Plan of Correction: The provider will provide the department with documentation the background check has been requested.
Standard 8VAC20-800-180-A
Based on record review, the provider did not obtain a current report of tuberculosis screening every 2 years for 1 caregiver.

Evidence:
1. The most current documentation of a tuberculosis screening in the record for Caregiver #1 was dated 05/06/2022.
Plan of Correction: The provider will call the physician's office to request the results of the most recent screening.

Based on record review, the provider did not review with the parent the emergency contact information required in the record annually and obtain the parent's signed acknowledgment of the review for 2 of 5 children.

Evidence:
1. The most recent documented review of the emergency contact information in the record for Child #1 was dated 01/03/2023.
2. The most recent documented review of the emergency contact information in the record for Child #3 was dated 03/01/2023.
