During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 01/15/2025, as a part of the provisional licensure period. The on-site inspection began at 10:45sm and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 20. The inspector reviewed 5 children?s records and 2 caregiver records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Repeat violation

The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. The physical exam in the record for Child #1 was obtained 5 days overdue.
Plan of Correction: The provider will ensure documentation of physicals are obtained before the child's 1st day or within 30 days of the first day.
Standard 8VAC20-800-60-B
Providers are required to maintain an up to date record for each child that contains all required information.
The record for Child #1 was missing the name, address and telephone number of each custodial parent's place of employment. Child #1 was present during the inspection and had been in care for 2 months.
The record for Child #3 was missing the policy number of the child's medical insurance. The child was present during the inspection and had been in care for 9 months.
The record for Child #4 was missing the address of two designated persons to contact in case of an emergency if the parent cannot be reached. Child #2 was present during the inspection and had been in care for 9 months.
Plan of Correction: The provider will request the information from the parents today.
Standard 8VAC20-800-80-C
Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child and document the method of verification. Proof of identity documentation in the records for Child #1 and #3 did not contain the date the documentation was viewed and the person who viewed the documentation.
Plan of Correction: Corrected during inspection.
Standard 8VAC20-800-800-B
The provider is required to review the emergency plan at least annually and update the plan as needed, documenting in writing each review and update to the plan.

The last documented review of the emergency plan was dated 11/09/2023.
Plan of Correction: The provider reviewed the plan during the inspection, confirmed no changes were needed, and documented the review of the plan.

Repeat violation

Before attendance, documentation of immunizations is to be obtained for each child. The documentation of immunizations in the record for Child #1 were a month overdue.
