During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 10/15/2025 as a part of the licensure period. The on-site inspection began at 10:55am and ended at 12pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 13. The inspector reviewed 5 children?s records and 4 caregiver/household member's records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Household member #1 was not completed and was overdue by two months.
Plan of Correction: Household member initiated the test and will return for the reading.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided in writing whether or not there is liability insurance on the family day home operation. The declaration of liability insurance form signed by Child #1's parents was blank, not indicating whether or not there is liability insurance. Child #1 had been attending the family day home for 3 weeks.
Plan of Correction: Provider will complete a new form for the parents with the correct information and obtain the parent's signature.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Records for children #2, #3, #4 and #5 did not have a parent?s signature for an annual review of the emergency contact information. The reviews for Child #2 and #3 were overdue by 4 months. The review for Child #4 was overdue by 3 months. The review for Child #5 was overdue by three weeks.
Plan of Correction: Provider reviewed information with parents and obtained parent signatures at pick up on the date of the inspection.
Standard 8VAC20-800-80-A
Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child and name and location of previous day care programs. This information was not available for Child #1, who had been in care for three weeks.
Plan of Correction: Provider will request from the parent.
Standard 8VAC20-800-90-A
Before attendance, documentation of immunizations is to be obtained for each child.
There was no documentation of immunizations for Child #1, who was in care during the inspection and had been in care for three weeks.
Plan of Correction: Provider has requested the information from the parent.
