During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on December 11, 2024. The on-site inspection began at 12:30pm and ended at 2:10pm. The inspector reviewed compliance in the areas listed above. There were six children present and three caregivers; the point total was 15. The inspector reviewed four children's records, three caregiver records, and one household member's record on-site. This inspection included document review, tour of the home, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 8VAC20-800-170-B
The initial tuberculosis (TB) screening for caregivers shall have been completed within the last 30 days and be signed by a physician, physician's designee or the local health department.

The initial TB screening for Staff #2 was more than 30 days old. Staff #2 provided a new TB screening that was completed 6 weeks after coming into contact with children.
Plan of Correction: I will make sure TB screening results are dated within 30 days prior to a new employee working with children.
Standard 8VAC20-800-680-A
Caregivers shall wash their hands with liquid soap and warm running water before feeding or helping children with feeding, and after diapering a child or assisting a child with toileting.

Caregiver #2 did not wash their hands with liquid soap and warm running water after diapering a child. Caregiver #3 did not wash their hands with liquid soap and warm running water before feeding or helping children with feeding.
Plan of Correction: I will remind my caregivers to wash their hands immediately as required. Violation was corrected during inspection.
Standard 8VAC20-800-830-D
The records of emergency evacuation shall be maintained for one year.

The emergency evacuation practice was not documented for October and November 2024.
Plan of Correction: I will fill out the dates for the October & November drills on m sheet . I have my December practice next week. I will make sure to document all future practice drills immediately.
