During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced on-site monitoring inspection was initiated and completed on September 2, 2025. The on-site inspection began at 2:00pm and ended at 4:00pm. The inspector reviewed compliance in the areas listed above. There were five children present and three caregivers; the point total was 16. The inspector reviewed five children?s records and two caregivers records on-site. This inspection included document review, tour of the home, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 04/07/25, were not posted in a conspicuous place on the premises.
Plan of Correction: Did not realize I had not posted the most recent inspection. I will print and post the inspection report the day I receive it. Violation corrected during inspection.
Standard 8VAC20-800-340-C
The bathroom shall be kept clean and contain a working toilet and sink, toilet tissue, liquid soap, and paper towels. The staff were not using paper towels to dry their hands after washing them.
Plan of Correction: Cloth towel was removed and informed staff they must use disposable towels at all times even for themselves. Violation corrected during the inspection.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Records for child #4 did not have a parent?s signature for an annual review of the emergency contact information. Child #4 had been in care for over a year.
Repeat Violation
Plan of Correction: Parent reviewed and corrected today at pick-up. I will be more attentive to annual checks. Violation was corrected during the inspection.
Standard 8VAC20-800-90-C
Documentation of additional immunizations is required once every six months for children under the age of two years. Child #2, eleven months old, last documented immunization was over seven months ago.
Repeat Violation.
Plan of Correction: Emailed parent that an updated immunization must be received by 9/16/25.
