Inspector Notes

A Code compliance inspection was initiated on 12/14/2022 and concluded on 12/14/2022. There were 14 children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed. The inspection started at 12:15 pm and concluded at 2:13 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review the center failed to ensure that all staff submit to a Field print fingerprint background check prior to first day of employment.
Evidence:
Staff #1, 2, 3, 4, 5, 6 do not have a fieldprint background check. All six staff have hire dates from 9/2020 to 9/2022. All six staff are working with the children as staff or substitutes.
Plan of Correction: The Pastor and preschool Director will ensure that all staff submit a Fieldprint fingerprint background check. These appointments will be scheduled as much as possible during the holiday break.

Based on record reviews the center failed to ensure that Prospective employee, volunteer, or any other person who is expected to be alone with one or more children enrolled in the religious exempt child day center except a parent-volunteer, or a parent or guardian who may be left alone with his or her own child WHAT: Sworn statement or affirmation WHEN: Before employment or commencement of service at the facility WHAT: Search of central registry and criminal history record check, as requested by the individual WHEN: Within 30 days of employment or commencement of service.
Evidence:
1. Staff #1, 2, 3, 4, 5, 6 do not have a sworn statement or affirmation on file. All staff have been hired more than 30 days and are currently working with the children.
2. Staff #3, 4, 5, 6 do not have a central registry check on file. All staff have been hired more than 30 days and are currently working with children.
Plan of Correction: The Pastor and preschool Director will have all staff complete a sworn statement and obtain a central registry check on the staff that do not have one on file.
