During the inspection, the inspector reviewed the areas listed above to include Code found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the Code reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site Code monitoring inspection was initiated and completed on 3/19/25. The on-site inspection began at 8:55 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 11 children present and three staff. The inspector reviewed four children?s records and eight staff records on-site. This inspection included: document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must request a search of the central registry prior to the employee's first day of employment.
Staff #8, who had been employed for one week, did not have a central registry search requested. Staff #1 was working alone with children.
Plan of Correction: The director registered with OBI and will request the background checks. All background checks will be requested prior to hire.
Standard 22.1-289.058
Each building that was built before 2015 and that houses a child day program that preschool-age children shall be equipped with at least one carbon monoxide detector.
The preschool area does not have a carbon monoxide detector.
Plan of Correction: Maintenance will purchase and install a carbon monoxide detector on the floors preschool children are present.
