During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 10/21/24. The on-site inspection began at 11:00am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 12 daycare children present, one household member child, and three caregivers; the point total was 41. The inspector reviewed five children?s records and five caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4As of the date of the inspection, the subsequent national criminal history background check results for Household Member #1 were six days past due.
The physical on record for Child #2 was obtained more than two months after the child?s first day of attendance.
The provider did not obtain verification of the identity and age of Child #2 who has been attending the family day home for more than five months.
The immunizations on record for Child #2 were obtained more than two months after the child?s first day of attendance. The immunizations on record for Child #3 were obtained three days after the child?s first day of attendance.
