During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/15/25. The on-site inspection began at 9:10am and ended at 10:05am. The inspector reviewed compliance in the areas listed above. There were nine children present and three caregivers; the point total was 25. The inspector reviewed four children?s records, three caregiver records, and one household member record on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/4/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-180-A
The provider is required to obtain for each caregiver and adult household member a current tuberculosis (TB) test or screening every two years from the date of the first screening.

The TB screenings on record for Caregiver A and Caregiver B were no longer current and were more than five weeks past due for an update.
Plan of Correction: TB test for Caregiver A and B were conducted and a copy of result kept in the files.

Child-resistant protective covers are required to be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

On 10/15/25 an outlet was observed to be uncovered directly behind the crib of a 12-month-old child who had been napping in the crib at the beginning of the inspection. The outlet was within reach of the child when they were in their crib and the crib is behind a closed door when children are napping.
Plan of Correction: Protective cover installed on unused outlet immediately.
