Inspector Notes
An unannounced monitoring inspection was conducted on October 3, 2023 from approximately 12:15 pm to 1:15 pm. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, caregiver training, preventing the spread of disease and nutrition. The provider record, Assistant record, 1 household member record and 10 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. The provider, assistant, and 10 children were in attendance for a total of 25 points. The children were observed finishing up lunch, getting diapers changed, and taking a nap while the inspector was on the premises.
The first aid kit was inspected and found complete. The menu, emergency numbers, and daily schedule were posted.
The information gathered during the inspection determined violations with applicable standards or law.

4Evidence: Child #3, age 4 months, was observed sleeping in a crib on top of a "Docatot", a u-shaped pillow that surrounds a child.
Evidence:
The record for child # 9, date of enrollment 08/29/22, did not contain documentation that verification of identity was viewed.
Evidence: When asked to view documentation of emergency evacuation and shelter-in-place drills the provider was unable to locate the documentation.
Evidence: The record for child #3, date of enrollment 06/19/23, indicated that the parents were opposed to immunizations due to religious tenets or practices. The record did not contain an affidavit on the current form approved by the Virginia Department of Health.
