During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/05/2026. The on-site inspection began at 2:52 pm and ended at 4:43 pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 27. The inspector reviewed 9 children?s records and 6 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/23/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical exam record available for child #4 who has been in attendance for 4 months.
Plan of Correction: Child's records will be view to ensure all documents needed has been received.
Standard 8VAC20-800-180-A
Repeat Violation
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for household member #3 was not completed and was overdue by 1 year.
Plan of Correction: Household member #3 has his own residence. Asking for him to be removed.
Standard 8VAC20-800-210-A
Repeat Violation
Caregivers shall obtain a minimum of 16 clock hours of training annually.

1. Caregiver #1 obtained 10.5 training hours in 2025.
2. Caregiver #3 did not obtain any training hours in 2025.
Plan of Correction: Caregiver #1 will complete the minimum training required by end of each calendar year.
Standard 8VAC20-800-600-B
Infants and toddlers shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment.

1. Upon the inspector's arrival at 2:52 pm, Child #4, age 6 months, was awake and confined in a swing seat. Child #4 remained awake in the swing seat until 4:05 pm when the child's parent arrived.
2. When the inspector arrived at 2:52 pm, child #6, age 13 months, was confined in a high chair eating snack. When snack was over at approximately 3:30 pm, child #6 remained in the high chair until 4:15 pm. Child #6 was taken out of the high chair to have their diaper changed and it was discovered that child #6 had vomited. After cleaning up child #6, they were placed back into the high chair.
3. When the inspector arrived at 2:52 pm, child #3, age 8 months, was confined in a high chair eating snack. When snack was over at approximately 3:30 pm, child #3 remained in the high chair until approximately 4:40 pm. The high chair was moved from the kitchen to the living room with the child still in it and put in front of the television to watch a program.
Plan of Correction: Infants/toddlers will not be in a crib, high chair or swing for more than 30 minutes at a time. One staff will ensure the child is free to move around safely.
