Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated and concluded on April 16th, 2021. The provider was contacted by telephone to initiate the inspection. There were 6 children present and 2 staff. The inspector reviewed 2 children's records, 1 caregiver's record and 2 household members records submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined violations of applicable standards or law and are are listed on the violation notice. Evidence: The provider's most recent central registry background check expired 12/15/2020. Household member #1 and #2's most recent central registry background checks expired 12/14/2020.
