Inspector Notes

An unannounced renewal inspection was conducted with the provider beginning at 9:30 AM and ending at 11:10 AM. There were four children, ages 2, 3, 4, and 5 years old, in the care of the provider for a total of 7 points. The children were observed playing games and with toys. Three children's records were reviewed. Three household records were reviewed and in compliance. The emergency drill log was reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance.

Based on the review of three children's records, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child A, start date 1/29/18, was missing documentation of a physical examination from their record.
Plan of Correction: The provider stated that she would request the missing documentation from the child's parent.
