Inspector Notes

An unannounced, monitoring inspection was completed on 11/22/2024. The inspection began at 8:30 am and ended at 9:36 am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregivers; the point total was 6. The inspector reviewed 2 children?s records and 1 caregiver and 2 household member records. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/06/2024. A POC submitted after this date will not appear on the public website.

The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

The most recent tuberculosis screening for household member #1 was more than 6 months overdue.
