Inspector Notes

An unannounced renewal inspection was conducted 1/30/2023 from 9:35am through 11:10am. There were 54 children directly supervised by 11 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 5 staff records, 5 children?s records, emergency drills, injury reports, emergency supplies, and policies were inspected. There were reportedly no medications on site. Children were observed completing puzzles, coloring, practicing writing, playing with blocks, building a Mr. Potato Head, tracing with stencils, and sorting by color. Infant care including feeding was also observed. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not request a search of the child abuse and neglect registry or equivalent registry from any state in which staff have resided in the preceding five years within 30 days of hire.

Evidence: Staff #3 (Date of Hire: 9/21/2022) has resided in Kentucky and Nebraska within the last 5 years and did not have documentation on file of a request to search the Kentucky and Nebraska child abuse and neglect central registry.
Plan of Correction: Out of state and Virginia criminal history checks were completed. Virginia Central Registry was completed. The central registry for out of state was submitted 1/31/23. The employee will not be left alone with children until report is received. 1/2 of the reports was received 2/2/23.
