Inspector Notes

An unannounced monitoring inspection was conducted 9/11/2023 from 11:00am through 1:10pm. There were 48 children directly supervised by 8 staff. The physical plant, outdoor playground area, programming, menus, 5 staff records, 5 children?s records, emergency drills, injury reports, emergency supplies, and policies were inspected. There were reportedly no medications on site at the time of inspection. Children were observed eating lunch, playing outside on the playground, and preparing for nap. Lunch served today was beef stroganoff, carrots, mixed fruit, and milk. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. The center was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection.

Based on record review, the center did not obtain the results of a Virginia Child Abuse and Neglect Central Registry search for all staff within 30 days of employment.

Evidence: Staff #4 (Date of Hire: 5/22/2023) had Central Registry results on file dated 8/9/2023. Staff #4's last day of employment with the center was 8/31/2023.
Plan of Correction: The Virginia Child Abuse and Neglect Central Registry search was sent on 3 separate occasions.
Standard 8VAC20-780-130-A
Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: Child #1 (Start Date: 8/28/2022) was present at the time of inspection and did not have immunizations on file.
Plan of Correction: Children will not be able to start without required paperwork. All paperwork was received on September 22, 2023.

Based on record review, the center has not practiced lockdown procedures at least annually.

Evidence: The most recent lockdown drill was conducted on 1/11/2022.
Plan of Correction: Lockdown drill will be completed by October 2, 2023.

Based on documentation review and staff interview, the center did not maintain documentation of all emergency evacuation drills.

Evidence: There was no emergency evacuation drill documented for the month of August 2023. The most recent emergency evacuation drill was documented as 7/28/2023.
Plan of Correction: A fire drill was completed on August 12, 2023. All fire drills will be documented immediately after occurrence.

Based on record review, the center did not have all required information on file for each enrolled child.

Evidence: Child #1 (Start Date: 8/28/2023) did not have documentation of the center viewing the child's proof of age and identity on file.
