Inspector Notes
A complaint investigation was completed on 9/11/2023 from 11:00am through 1:10pm. The complaint consisted of allegations received by the Northern Licensing Office on 9/5/2023 regarding staff-to-child ratio requirements. Based on staff interviews and observations, the complaint regarding ratio was determined to be valid. Areas of non-compliance are identified in the Violation Notice.

Based on observation, the center did not maintain the staff-to-child ratio of one staff for four children for children aged birth to 16 months.
Evidence:
1. In the Older Infants room there were five infants with one staff. The youngest child in the room was 10 months old.
2. In the Younger Infants room there were five infants with one staff. The youngest child in the room was 4 months old.
