During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 5, 2025. The on-site inspection began at 11:30am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 56 children present and 11 staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

In the School Age classroom, the light switch adjacent to the stage was missing a cover, exposing the wall's interior and screws.

The Preschool and Prekindergarten playground had chipped, peeling paint on top of the fence railings.

On the Infant, Toddler, and Two's playground, there was a small screw sticking out in the back of the hose box on the wall.
Plan of Correction: The light switch was covered, and the screw was fixed 8/5/25. The fence material was ordered 8/6/25.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

The School Age classroom did not have a written allergy list, while children were present in the classroom.
Plan of Correction: The Allergy List was taken down the day of the visit to be updated. It was updated 8/5/25.
Standard 8VAC20-780-550-I
A 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone.

Local emergency and poison control numbers were kept inside classroom binders and not posted in visible places at each telephone.

Standard 8VAC20-780-560-F
When centers choose to provide meals or snacks, a menu listing foods to be served for meals and snacks during the current one-week period shall meet all requirements.

The center's menu was not dated and was posted on the kitchen refrigerator, not in a conspicuous location or given to parents.
Plan of Correction: Menus are sent to parents via the Parent Portal and posted in the foyer. Menus were dated and posted in each classroom.
