Inspector Notes

Licensing Inspector Anderson conducted an unannounced complaint inspection in response to allegations received by the licensing office on April 24, 2023, in the areas of supervision, ratios, facilities and neglect. The investigation of the complaint was initiated on April 24, 2023 and findings were completed on June 15, 2023.

As the information gathered during the investigation does not support the allegations, the complaint is determined to be ?NOT VALID.?

Violations found during today's inspection were unrelated to the complaint allegations and are listed on the violation report.

Based on inspection, the Vendor failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

EVIDENCE:
Written documentation of daily attendance for the school age children was unavailable.
Plan of Correction: Staff will document arrival and departure for each child as it occurs.

Based on observations, the Vendor failed to ensure staff members wash their hands with soap and running water before and after changing a diaper.

EVIDENCE:
Licensing Inspector observed Staff Member #1`wash their hands with soap and warm running water before and after changing a 2 year old's diaper.
Plan of Correction: Staff will wash their hands before and after diapering a child.

Based on observations, the Vendor failed to ensure diapering surface is sanitized after each use.

EVIDENCE:
Licensing Inspector observed Staff Member #1 during diapering; the diapering surface was not sanitized after each use.
Plan of Correction: Staff will clean diapering surface with soap and water & sanitize after diapering each child. Corrected during inspection.
