An unannounced inspection was initiated on 06/16/2025 and completed on 07/25/2025 in response to a complaint received by the licensing office on 06/16/2025 relating to allegations of the subsidy program vendor requirements. The inspector reviewed 1 child?s record on-site. The inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); there for the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
