Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 07/25/2025 and completed on 08/11/2025. The on-site inspection began at 10:00 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 98 children present with 22 staff. The inspector reviewed 9 children?s records and 9 staff records both on-site and electronically. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined that non-compliance with applicable code sections, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/18/2025. A POC submitted after this date will not appear on the public website.

For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee within 30 days of employment.

The central registry for Staff 1 was obtained 21 days after the 30th day of employment.
Plan of Correction: All future employees will have central registry prior to hiring. This will be checked by director and secretary.
