A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/18/2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION

For each group of children, the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

The attendance in Classroom 2 did not document the departure of each child as it occurred on 07/24/2025. The attendance in Classroom 2 was not taken on 07/25/2025.
Plan of Correction: Staff member has been informed of how to correctly sign children in and out again.

REPEAT VIOLATION

Hazardous substances, such as cleaning materials, insecticides and pesticides, shall be kept in a locked place using a safe locking method that prevents access by children.

There was a cleaning closet in Building 1, which contained various cleaning materials and pesticides, that was unlocked and open; the supplies inside were accessible to the children. The cleaning supply cabinet in Classroom 1 was broken and unlocked; the supplies inside were accessible to the children. There were cleaning materials sitting on a stack of chairs in Classroom 8 that were accessible to the children.
Plan of Correction: All cleaning supplies have been secured with a lock. Staff have been instructed to keep all supplies locked up.

Cleaning and sanitizing materials shall not be located above food, food equipment, utensils, or single-use articles.

There were various cleaning supplies stored above food equipment and utensils in the unlocked storage closet in Building 1.
Plan of Correction: Cleaning supplies have been stored separated from food equipment and utensils.

Children less than 10 years of age shall always be within actual sight and sound supervision of staff.

Child 1, age 7 years, was observed receiving services from an outside organization out of sight and sound supervision of center staff.

Toilet chairs shall be emptied promptly and cleaned and sanitized after each use.

The toilet chairs in the women?s restroom of Building 1 that contained urine; the toilet chairs were not promptly cleaned and sanitized after each use.
Plan of Correction: Staff have been informed to empty and sanitized after each use.

Evacuation procedures shall be practiced, at least monthly.
