Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.
A renewal inspection was initiated on 07/27/2021 and concluded on 08/02/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 10 children, ages 20 months-11 years, totaling 14 points, with one care giver supervising present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, household members, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 2 children?s records, 1 caregiver record, 1 household member record and 2 pet records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
2Evidence: During the virtual inspection Child #1 was observed using a pillow during naptime. Record review determined that Child #1 was 20 months old.
Evidence:
1. The record for Child #1 did not contain the following components: the telephone number of each custodial parent?s place of employment, the office address and phone number of the child?s physician, the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (the address for the first designated person was in complete and there was not a second designated person provided), the policy number of the child?s medical insurance (an insurance provider name was provided), and the name and address of previously attended child day care and schools.
2.
