Inspector Notes

An unannounced renewal inspection was conducted on 08/07/2023. There were 6 children, ages 5 years ? 11 years, totaling 5 points and 1 caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment, and equipment, preventing the spread of disease, emergencies, and nutrition. A total of 3 children?s records, 1 caregiver?s record, 1 household member?s record and 3 pet records were reviewed. The children were observed during center time and lunch time. The inspector arrived for the inspection at 11:55am and departed at 1:29pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-800-100-A
Based on the review of three children?s records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance.

Evidence:
1. Child #1 first date of attendance was 06/06/2022 and on the day of the inspection the did not have documentation of a physical examination.
2. Child #2 first date of attendance was 05/15/2023 and on the day of the inspection the did not have documentation of a physical examination.
Plan of Correction: Provider will obtain physical records from parent.

REPEAT VIOLATION

Based on discussion and record review, the provider failed to maintain documentation of the current rabies vaccination.

Evidence: The most recent rabies vaccination for Pet #3 expired on 7/14/2023 and on the day of the inspection a current rabies vaccination was not documented.
Plan of Correction: Provider will obtain current rabies vaccination for pet.
