Inspector Notes

An unannounced, on-site renewal inspection was initiated on 8/6/2024 and the inspection concluded on 8/6/2024. The on-site inspection began at 12:57PM and ended at 1:45PM. The inspector reviewed compliance in the areas listed above. There were 4 children and 1 caregiver present. The inspector reviewed 4 children?s records, 1 caregiver record and 1 household member record on-site on 8/6/2024. This inspection included document review, tour of the facility and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/13/2024. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION

The provider shall obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child 4 who was provided care in August 2024. Child 4 has been enrolled for 2 years and 4 months.
Plan of Correction: Provider will obtain physical record from parent.
Standard 8VAC20-800-110-B
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physicians designee, or an official of the local health department. The immunization record for Child 4 was not signed by a physician who was provided care in August 2024. Child 4 has been enrolled for 2 years and 4 months.
Plan of Correction: Provider will obtain signed immunization record from parent.

The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan. The most recent update to the emergency plan was dated 3/1/2023 during the inspection on 8/6/2024.
