Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/12/2025 and completed on 3/12/2025, as a part of the licensure period. The on-site inspection began at 3:15PM and ended at 4:11PM. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 4. The inspector reviewed 4 children?s records on-site on 3/12/2025. This inspection included - document review, tour of the facility and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child 3 and Child 4, enrolled for 5 years, who were provided care in March 2025.
Plan of Correction: Provider will obtain the physical record for each child.
