Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 15, 2024. The on-site inspection began at 9:00am and ended at 10:00am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregivers; the point total was 34 The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis screening is required every two years from the last screening.

An updated tuberculosis screen for caregiver #1 was not completed and was overdue by two weeks.
Plan of Correction: The TB screening will be completed and results will be placed in their record.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child's record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.
The records for children #1 and #2 did not have a parent's signature for an annual review of the emergency contact information. Child #1 has been in care for over 4 years without an annual review for over 9 months. Child # 2 has been in care for 2 years and has not had an annual review for 21 months.
Plan of Correction: I will make sure all parents review their child's record and sign off on it.
