Inspector Notes

A renewal inspection was initiated on February 14, 2023 and concluded on February 15, 2023. There were thirteen children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review and interview, facility did not ensure required out of state background checks are completed when a staff member has lived outside the state of Virginia within the past 5 years.

Evidence: The record for staff #4 (start date 10/3/2022) had documentation of living outside the state of Virginia within the past 5 years and there was not a completed child abuse and neglect registry for that state. Director confirmed a incorrect search request was submitted.
