Inspector Notes
An unannounced monitoring inspection was conducted on January 30, 2024 from approximately 3:10 pm to 4:40 pm. Two staff were present supervising eight school age children. Children were observed participating in group led games and table activities. Four staff and five child records were reviewed. Violations were found and are noted on the violation notice.

Evidence: The records for staff #1 (start date 5/23/2023) and staff #2 (start date 5/23/2023) did not have a completed central registry search. Site Director confirmed the results letter could not be located.

Based on review, facility did not ensure for each group of children there is a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: The tablet used for maintaining attendance of children was not operable during inspection. The written attendance log documented 7 children present when 8 children were observed present.
