Inspector Notes

An unannounced monitoring inspection was conducted on January 17, 2023 between 12:50pm and 2:35 pm. There were 11 children present with two (2) caregivers supervising within the supervision guidelines. The children were observed preparing for nap, toileting, and handwashing procedures. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, fire drill log, menu, attendance, child and caregiver records, and emergency procedures. There was no medication on sight at the time of inspection.

Based on record review, the provider did not obtain documentation for a 5 year repeat check.
Evidence:
1. Caregiver #1's record does not have documentation of an updated central registry search (expired 1/5/18)
2. Caregiver #2's record does not have documentation of an updated central registry search (expired 1/5/18).
Plan of Correction: I will submit online tonight and send you the documentation it has been submitted.

Based on record review and interview, the provider failed to obtain from the parent verification of the identity and age of the child within seven business days of the child's first day of attendance.
Evidence:
1.Child #2 (start date 8/24/22) does not have proof of ID on file.
2.Child #5 (Start date 10/4/22) does not have proof of ID on file.
3. Provider confirmed verification of the identity and age was not obtained.
Plan of Correction: I will contact the parent today and ask them to bring it today when they pick up.
