REPEAT VIOLATION

The provider is required to obtain a completed sworn statement or affirmation disclosing whether household members age 18 years or older have ever been convicted of or is the subject of any pending criminal charges for any offense within or outside the Commonwealth and whether or not they have been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

At the time of inspection, there was no sworn statement completed for Household Member #3, who has lived in the family day home for 30 days.

REPEAT VIOLATION

Providers must obtain a completed national criminal background fingerprint check within the 30-days of a new house hold members arrival.

Household Member #3's fingerprint check was completed 39 days after the household member began residing in the home

REPEAT VIOLATION

Household members 14 years of age and older must obtain the results of a central registry search by the end of the 30th day of residence.

Household Member #3 did not have central registry results during the inspection and has been residing in the home for over 30 days.

Lockdown procedures shall be practiced at least annually.

Caregiver #2 confirmed that a lockdown drill was not completed for 2025.

The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

The provider did not obtain documentation of a physical examination for Child #6. Child #6 has been enrolled for over nine months.

Providers and substitute providers shall have a current certification in cardiopulmonary resuscitation (CPR) and first aid, as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

Caregiver #1 and #3 did not have a current CPR certification obtained through an approved organization at the time of the inspection.

Providers and substitute providers shall have a current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department.

Caregiver #1 and #3 do not have a current first aid certification obtained through an approved organization.

An assistant who is left alone with children in care shall have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

Caregiver #2 does not have current certification in first aid from an approved organization.

The provider shall must obtain from each adult household member prior to coming into contact with the children, a current Report of Tuberculosis Screening (TB) documenting the absence of TB in a communicable form.

There was no TB screening documenting the absence of TB for Household Members #1, #2, and #3, who have been residing in the family day home for over 30 days.

Hanging items, including appliance cords, shall be out of reach of children under five years of age.

At the time of the inspection, there were cords for a telephone and mounted camera within reach of Child #1, 2, 3, 4, and 5, who are all under five years of age.

Potentially poisonous materials shall be stored in an area inaccessible to children.

Poisonous cleaning chemicals were observed in an unsecured cabinet under the kitchen sink, accessible to children.

Four cosmetic products, lotion, cologne, hand sanitizer (label states to keep out of reach of children), and a shower cleaner were observed in two bathrooms, accessible to children. Caregiver #2 confirmed that both bathrooms are used by the children.

Sharp kitchen utensils and other sharp objects shall be inaccessible to children.

Sharp knives were observed in an unsecured kitchen drawer, accessible to children.
