Inspector Notes

An unannounced monitoring inspection was conducted on 01/11/2024 from 11:20 am - 1:00 pm. At the time of entrance there were 10 children in care with 2 caregivers. The ages of children in care counted for a total of 27 points.
Children were observed during transition from play to lunch. Lunch and transition to nap was reviewed in addition to diapering, toileting and hand washing procedures.
Records reviewed for 7 children and 5 adult caregivers/household members.

One violation was found in the area of administration. This violation is listed on the violation notice issued to the FDH.

Standard 8VAC20-800-180-A
Based on record review and interview, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Evidence:
1. The most recent TB screening for caregiver 2 has not been updated every two years.
a. Written documentation on file indicates the most recent TB screening as being conducted on 05/07/2021.
2. Caregiver 2 and the provider confirmed that the TB screening had not been updated.
Plan of Correction: The provider stated that caregiver 2 will update the TB screening within the next two weeks.
