During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 03/23/2026 and completed on 03/23/2026. The on-site inspection began at 9:50 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 29. The inspector reviewed 10 children?s records and 5 caregiver/household member records on-site on 03/24/2026.
This inspection included:

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-250-F
Caregivers who work directly with children shall annually attend the department's health and safety update course.
Caregiver 1 and caregiver 2 have not completed the subsidy 3 hour annual update training for 2025.
Plan of Correction: The provider stated that both caregivers will complete the 3-hour annual preservice training over the weekend.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening.
The most recent screening for tuberculosis for caregiver 1 was completed more than 30 months ago.
Plan of Correction: The provider stated that caregiver 1 will obtain an updated TB screening the first week of April 2026.
Standard 8VAC20-800-530-B
Cribs when being used by infants shall have a tight-fitting bottom cover.
The crib sheet in the crib assigned to child 1 was loose fitting.
Plan of Correction: The provider stated the loose fitting crib sheet was removed immediately before nap.
Standard 8VAC20-800-540-A
Infant carrier seats, swings, strollers, feeding or activity tables, and high chairs shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.
Child 1 was observed sitting inside of an infant feeding seat without the safety strap securely fastened.
Plan of Correction: The provider stated that children will use floor chair with straps at all times.
