Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 10:00am through 11:15 am. There were a total of 11 children (25pts) observed with two caregivers. A total of five children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, bathroom breaks, handwashing, eating, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard 8VAC20-800-100-A
Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1's record (Start date-08/17/2022) did not contain documentation of a physical examination.
2. Caregiver #1 stated that the health examination was obtained from the parents, but was unable to locate it at the time of the inspection.
Plan of Correction: The provider will look for the physical examination. If the provider cannot locate the physical examination, they will obtain a copy from the parents.
