Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 10:45 am. There were a total of 13 children (29 points) observed with two caregivers. A total of six children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, games, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Violations
4Evidence:
1. The family day home has a capacity of 12 children. At the time of the inspection, there were a total of 13 children(29pts) observed with two caregivers.
Evidence:
1. Child # 1's record (Start date-08/14/2023) did not contain documentation of immunizations.
2. Caregiver #2 confirmed that Child #1's immunization records were obtained before the first date of attendance, but the copy was returned to the parent for proper certification by a physician.
Evidence:
1. Child #7's (2 years old) hands were not washed with soap and warm running water after diapering.
Evidence:
1. At the time of the inspection, documentation of emergency evacuation and shelter in place drills was not available for review.
2. Caregiver #1 stated that documentation for the emergency evacuation and shelter in place drills was maintained, but could not be located at the time of the inspection.
