Inspector Notes
An unannounced monitoring inspection was conducted on February 8, 2024 between the hours of 9:15 am through 10:30am. There were a total of 10 children (23 points) observed with two caregivers. A total of six children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, games, practicing letters, bathroom breaks, hand washing, and other organized activities. 1. Caregiver #2's record did not contain a current Tb screening. Documentation of the last Tb screening was dated 09/14/2021.
2. Caregiver #2 confirmed that a current Tb screening was not conducted.

1. Child #2's record (start date-11/01/2021) did not contain written authorization if a caregiver is to administer prescription or non prescription medication.
