Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on January 24, 2025, as a part of the licensure period . The on-site inspection began at 9:15amand ended at 10:15am. The inspector reviewed compliance in the areas listed above. There were 10 children present and two caregivers; the point total was 22. The inspector reviewed six children?s records and three caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The parents of Child #1were not provided in writing the Provisions of the family day home's emergency preparedness and response plan. Child #1 has been in care for five months.
