An unannounced inspection was initiated on 3/2/26 and completed on 3/24/26, with local agencies, in response to a complaint received by the licensing office on 3/2/26 relating to allegations of staffing and supervision.
This inspection included: document review; tour of the facility, interviews.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law.
