Inspector Notes
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on January 22, 2024 regarding allegations of supervision, building maintenance and behavioral guidance. The inspection was conducted on March 14, 2024 from approximately 8:00 AM to 10:44 AM. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

Based on observation, the center did not ensure that the temperature in areas occupied by children is maintained no lower than 68* F.
Evidence:
1. Two of the three wall mounted thermostats that control the temperatures in the center showed a temperature reading of 65*F and 66*F.
2. The administrator stated that the thermostats do not work correctly.

Based on observation and interview, the center did not ensure to maintain the required staff-to-children- ratio of 1 staff to 5 children in the preschool classroom.
Evidence:
1. The center currently has a temporary staff to child ratio / group size waiver, approved on 9/21/23 which allows a ratio of 1:6 for children 16 months to 24 months.
2. On 3/14/2024, 7 children were observed in room #2, with one staff member. Children in this room were 19 months, 18 months, 19 months, 18 months, 22 months, 2yrs and 2yrs old.
3. The administrator #1 arrived at approximately 9:00 AM. At approximately 9:04 AM, administrator #1 moved one child to another classroom.
