Inspector Notes
An unannounced complaint investigation was conducted today in regards to a complaint received by the Fairfax Licensing Office on 9/19/2023 in regards to Emergency Procedures and Medication Administration. Interviews were conducted and caregiver training was reviewed.
A preponderance of the evidence supports the allegation and it is deemed ?valid?. Violations related to the complaint can be located on the Violation Notice.

Violations
3Evidence:
On 5/26/2023, petroleum jelly was applied to a laceration on Child 1?s forehead. Child 1 did not have a written authorization on file permitting the use of petroleum jelly.
Evidence:
1. On 5/26/2023, Child 1 hit their head on a rocking chair resulting in a laceration on the child?s eye brow that was deep enough to require stitches.
2. There is no record of this injury in the child?s record.
Evidence:
No report was ever made to OCCHS of an injury sustained by Child 1 on 5/26/23 that resulted in the child needing stitches.
