During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was completed on August 6, 2025, as a part of the licensure period. The on-site inspection began at 1:00pm and ended at 2:10pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers; the point total was 9. The inspector reviewed 4 children?s records and 2 caregiver/household members records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-520-A
Cribs shall be provided for children from birth through 12 months of age.

The licensing inspector observed two children sleeping in cribs at the time of inspection. The children were 15 months and three years of age.
Plan of Correction: I will have all children over the age of 12 months sleep on the cots and rest mats.
Standard 8VAC20-800-780-B
The provider will review the emergency contact information with the parent annually.

There was no annual review of the emergency contact information for Child A.
Plan of Correction: I will have the parent review the emergency contact information.
Standard 8VAC20-800-90-C
The provider will obtain documentation of additional immunizations for children in care.

Child A, B and D did not have current immunization documentation in the records.
Plan of Correction: I will have the parents bring in the documentation.
