During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 31, 2026 and completed on March 31, 2026. The on-site inspection began at 9:30am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 20. The inspector reviewed 4 children?s records and 1 caregiver/household member(s) records on-site on March 31, 2026.
This inspection included document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-180-B
Provider shall maintain a written hard copy of the record of daily attendance that documents the arrival and departure of each child.

The provider did not have documentation of the arrival times for all children in care on the day of inspection.
Plan of Correction: All records will be up to date.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to or within 30 days after the first day of attendance.

Child B and C had no documentation of a physical examination. They have been attending for over thirty days.
Plan of Correction: I will have to parents bring me the documentation.
