Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time.

An unannounced monitoring inspection was conducted at 11:24am. The provider was contacted by phone to initiate the inspection. There were 8 children present = 21 points with the provider and substitute assistant caregiver. Ratios and supervision were in compliance at the time of inspection. Children were observed eating lunch, which each child packs daily. The home was very well supplied with toys, books, and equipment for the children, and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Standard 22.1-289.035-A
Based on information received, it was determined that a substitute assistant caregiver did not have completed background checks prior to working with the children. Evidence: The substitute assistant does not have a completed criminal history check.
Plan of Correction: The fingerprinting will be completed.

Moving forward, it will be completed prior to working.
Standard 8VAC20-770-60-B
Based on information received, it was determined that a substitute assistant caregiver did not have completed background checks prior to working with the children. Evidence: The substitute assistant does not have a completed Sworn Disclosure Statement.
Plan of Correction: The Sworn Disclosure Statement will be completed.

Moving forward, it will be completed prior to working.
Standard 8VAC20-800-120-B
Based on information received, the record for a substitute assistant caregiver (working on the date of inspection) did not include required information. Evidence: Substitute Assistant - There was no documentation of: date of employment, emergency contact, 2 written reference checks, and background checks.
Plan of Correction: The inspector will provide a checklist of requirements to help with ensuring future compliance.
Standard 8VAC20-800-180-A
Based on records reviewed, it was determined that 4 of 4 adult records did not include documentaiton of TB screenings every 2 years.

Evidence:

1. Provider - The date of the last TB screening was 7/30/19.
2. Assistant #1 - The date of the last TB screening was 7/29/19.
3. Adult #1 - The date of the last TB screening was 8/21/19.
4. Adult #2 - The date of the last TB screening was 8/21/19.
Plan of Correction: TB screenings will be updated.

Moving forward, they will be completed every 2 years.
Standard 8VAC20-800-90-C
Based on records reviewed, it was determined that 1 of 2 children's records did not include documentation of immunizations every 6 months until the age of 2 years old. Evidence: Child #5 - The immunizations on-file were dated June 2021 by the physician, when the child was 3 months old.
Plan of Correction: The provider will request updated immunizations from the parents.

Moving forward, immunizations will be required every 6 months until the age of 2 years old.
