Inspector Notes
An unannounced monitoring inspection was conducted at 10:06am. Observed 8 children totaling 21 points with 2 caregivers. Ratios and supervision were in compliance. Children were observed playing with toys, finger painting and eating lunch. The caregivers were actively engaged with the children and activities. The daycare areas of the home were found to be clean and well supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found with multiple violations being repetitive and systemic. 1. The findings from the previous inspection were not posted. The violation notice from 8/11/22 was posted.
2. The most recently issued license was not posted. The license that expired in July 2023 was posted.
Based on records reviewed, 1 of 10 children's records did not include documentation of a physical exam within the first 30-days of attendance. Evidence: The record for Child #12 did not include a physical exam. The child began attending in September 2022.
Based on records reviewed, a caregiver's record did not include documentation of required information. Evidence: The record for Caregiver #4 did not include the date of employment; a minimum of 2 reference checks completed prior to the first day of working; and name, address and phone number of an emergency contact. Caregiver #4 reportedly began working 2 weeks prior.
Based on records reviewed, there was no documentation of a TB screening for a caregiver prior to coming into contact with the children. Evidence: There was no documentation of a TB screening for Caregiver #4 who was observed working with the children on the date of this inspection and reportedly began working 2 weeks prior.
Based on records reviewed, 3 of 10 children's records did not include required information.
Evidence:
1. Child #9 - There was no documentation of the parents receiving and signing the Information for Parents form.
2. Child #11 - There was no documentation of parents signing the emergency medical authorization form, emergency preparedness plan, and liability insurance statement.
3. Child #12 - There was no documentation of an allergy action plan for the diagnosed food allergy.
Based on records reviewed, 6 of 10 children's records did not include documentation that emergency contact information was reviewed and signed by parents annually.
Evidence:
1. Child #9-The last annual parent signature was documented as January 2023.
2. Child #10-The last annual parent signature was documented as October 2022.
3. Child #11-The last annual parent signature was documented as July 2022, when the child enrolled.
4. Child #12-The last annual parent signature was documented as April 2022.
5. Child #13-The last annual parent signature was documented as November 2022.
6. Child #16-The last annual parent signature was documented as February 5, 2023.
Based on records reviewed, 8 of 10 children's records did not include documentation of verification of the child's proof of identity using an acceptable form of identification. Evidence: The records for Child #9, #10, #11, #12, #14, #15, #16 and #18 listed a document that does not qualify as a proof of identity. The social security card was listed as the document accepted.
Based on records reviewed, 4 of 10 children's records did not include documentation of immunizations every 6 months until the age of 2 years old.
Evidence:
1. Child #10-The immunizations on-file were dated October 2022 when the child was 13 months of age.
2. Child #11-The immunizations on-file were dated July 2022 when the child was 13 months of age.
3. Child #12-The immunizations on-file were dated April 2022 when the child was 6 months of age.
4. Child #18-The immunizations on-file were dated June 2023 when the child was 8 months of age.
