8VAC20-800 Care of Children

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced focused monitoring inspection was initiated on 5/20/25 and completed on 6/6/25, with a local agency, in response to a self-report received by the licensing office on 5/19/25, relating to the supervision of children. The inspector reviewed 1 child's record and 1 caregiver record while on-site. These inspections included document review, tour of the outside areas of the home/neighborhood, interviews and video observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/19/25. A POC submitted after this date will not appear on the public website.

A caregiver shall provide direct care and supervision of each child at all times.

On 5/19/25, for a period of 1-2 minutes, a child under the age of 2 years old was out of sight and sound of the caregivers. Child #5 exited the backyard through a gate, without the caregivers knowledge, crossed the street and was found by Caregiver #1 in a neighbors yard.
